Booking an Appointment

Patient Registration

You may request an appointment with one of our offices by filling out the appointment registration form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. Within 2 business days an Administrator from one of our offices will contact you to schedule the appointment. If you have any questions, please do not hesitate to  contact us.

BOOK AN APPOINTMENT

To book an appointment for wisdom teeth removal, tooth extraction, dental implant or sleep apnea treatment, or any other dental concern, call the office you have been referred to (if you don’t have a referral, the office closest to you might be most convenient). You can also fill out our online form and we will get back to you as soon as we can, likely the same day. We look forward to hearing from you.
